Please state the purpose of the assistance, with dollar amount, and attach original receipts with back-up documentation, for reimbursement - OR - Provide Junior/Senior related bills/invoices to be paid to vendor. You may be required to provide a W9 from vendor and/or additional documentation, before we can pay. All receipts must be dated between October 1st to September 30th of the current fiscal year.
